Very nice art! :D I can't help but imagine Safiri, Ruby, and Esmeralda in my head as they appeared in my own story, so this helps me keep them straight. :p (Safiri is very close to my own, Ruby somewhat, and Esmeralda super different.)

I have honestly never liked 5ban's work at all, but I think their style is perfect for Nihilego; card looks great and has a nice, otherwordly feel to it.

You do need to be awake for it, but they give you drugs to help you relax. The procedure itself isn't painful at all, just a bit weird. It's also worth noting that specifically having a Trans PRK procedure, and also the cross-linking on top of it, is the most "unpleasant" (surgeon's words)...
