"Choose a card from your hand and put it on top of your deck. Search your deck for a Pokémon... shuffle your deck afterward."

Does the card I put on top of my deck stay on top of the deck after I shuffle or does it get shuffled back in to the deck?

It gets shuffled back into your deck. It's worded a bit stupidly, but the intent is that it gets shuffled in.
