Ruling Pokémon in the Lost Zone

I just want to make sure that this logic is correct.
A Ditto from boundaries crossed transforms into Hoppip and evolves into Skiploom from lost thunder and has a Klefki from steam siege equipped to it through its ability. The Skiploom uses its ability to put itself and all cards attached to it into the lost zone. This increases the number of cards in the lost zone by 4, those being ditto, Hoppip, Skiploom, and Klefki, correct?

Another question: a Ditto transforms into Klefki from Steam Siege and uses its ability to discard all cards attached to itself and attach it to a Pokémon. Does the ditto get discarded? Or become attached alongside the Klefki?

Yes, those four cards all go to the lost zone as Skiploom's text reads "Once during your turn (before your attack), you may search your deck for Jumpluff, put this Pokémon and all cards attached to it in the Lost Zone, and put that Jumpluff in its place. Then, shuffle your deck." That includes Pokémon from prior evolutions.

Another question: a Ditto transforms into Klefki from Steam Siege and uses its ability to discard all cards attached to itself and attach it to a Pokémon. Does the ditto get discarded? Or become attached alongside the Klefki?

The Ditto is discarded, per the Compendium:
Q. Say Ditto uses its "Transform" Ability to become a Klefki, then Klefki uses its "Wonder Lock" Ability to attach it to another Pokemon as a tool. When doing this what happens to the Ditto and Klefki cards?
