Name: Tybike
Badges: (Specify region and amount) Unova, 8
Trainer: Click Here for sprite
Background: same as MrGatr's trainer card
Anything Else?: (This is especially important as Trainer Cards are so open ended. If not specefied, I will use the same template as the above example)
can you have the number as 0402-1999-2100

@teegrun - Please use my form. The shop you got that from probably uses a different method to making trainer cards, so it would be difficult to go off of that form for me to make mine.
