Probably have to watch everything, including the epilogue. Double check if everything's completed, that's the only advice I've got.

Yeah, you're supposed to get an achievement and get the Mark VI armor and Legendary visor. Make sure you've completed each level itself on Legendary; loading rally points can skip ahead of segments you didn't complete on Legendary, and the game won't count it until you've beaten each rally point of each level on Legendary.
