It appears to be Industry Demi (or a very similar font, as I'm not sure what year it was designed). Stratum 1 Medium OT (with modified width) is also quite similar, though it was designed in 2004. I ran across a Japanese font family called "DF So Gei Japanese" and thought that may have been it, but the question mark has too many sharp bends. You can find character charts on each of the links provided.
EDIT: Actually, it looks like Eurostile Bold works fine at 110-112% width and strong rendering. I'm pretty sure it's a default font, too. That was a big fat waste of time, wasn't it?

What would be the correct way to word this in BW/XY format? Also, does the text in parenthesis need to be italic?

Here's an XY holosheet that I ripped from the scans:
Dodge (25%) over Screen (80%) seems to work fine on silver borders, but may need a bit of adjusting for backgrounds (depending on the lightness/darkness).

You can also use this for more of a balanced look:
Hard Light (10%) over Soft Light (100%) seems to work fine. Hard Light layer not required.
And yet another:
Soft Light or Overlay (70-100%)

While all of my XY holosheets will work, I feel that this one is the most accurate. Soft Light (100%) works fine for borders and some backgrounds, but others may require a bit of creativity. For instance, on the example below, I used Grain Extract 30% over Divide 50%. for the background.

Ok guys, sorry to poke a dead horse, but I got a new computer and I'm interested in making some image based fakes. Problem is, I have no fonts, and I'm not interested in looking through 40 pages for 5 or 6 fonts. Any help?

I'm kind of new to this whole faking thing, and I have a question. How would I apply a holosheet in GIMP? I'm not really sure how to do that.

I would do two things....1) duplicate the holosheet layer and set the new layer to Color Dodge. Mess around with the opacity; it usually depends on the background, and 2) bring the Blaziken image layer above the holosheet layer. You don't want it holo-tized, just the background.

I'm having an issue with fonts and alignment at the moment.
I am following the guide posted by Aschefield (thank you), but as you can see things don't seem to line up properly.
I'm using Photoshop (Elements 9) with the specified fonts (trusting that the versions I have, which I found in this thread, are correct) at the specified sizes with one of Aschefield's XY blanks. Despite this, the result isn't quite identical to what appears in the placement guide (even matching up identical bits of text, they are slightly off).
I can't seem to fix this, even after trying several different font downloads and other minute changes. Other things, like ability icons line up perfectly, so there's no issue there. Text is the only problem.
